For some reason after beating all 5 acts on insane it did not give me the "Commando" achievement. If you don't believe me just click on my sig and check it out.

I probably just have to beat the last act over again or something, but has anyone else seen or heard anything like this? It's not that big of a deal but it is just a little wierd.

BTW, I don't cheat. I'm not even sure they have cheats for GoW campaign.
